I have noticed the same thing at one of my sites - same MO, it's the same code that we have used for the last 8 years, also starting in 2.6 and we never had this before. We have one site that has heavy traffic (800 records a day added) and it has duplicated the key 3 times in the last 70,000 entries. It's a mystery - our code is similar to yours and the site is VFP 6.0 and Windows 2000.

If you, or anyone else, has any ideas, please let me know! We are not doing FLUSH commands but do plan on starting soon.
