My first job out of college was working for an insurance company, "Gulf Insurance Company". All accouting was done by computer. We would fill out coding sheets that were delivered to the IT department. They in turned would keypunch the code sheet, sort and run them to produce financial statement.

It seem like insurance company tend to remain well behind the curve when it comes to technology. I would not be surprised if some insurance company still operate in this manner.
