AFAIK, build just compiles the files VS.NET thinks have changed. Rebuild recompiles everything. I've had enough flakyness w/Build that I tend to use Rebuild all the time. Of course, as your projects get larger this might not be a great idea.

I think Tools->Options, then (Environment, General) Reset Window Layout might do what you want.
