Any idea why if I put the line:
<f href="/PDFs/MyPDF.pdf"/>
where it is locate in your example (just before the < /xfdf >, I receive the message:

Unable to locate the form, because the FDF lacks an /F key. If the FDF came back as a result of a submit from a PDF form, then the URL is probably missing "#FDF" at the end.

However, if I locate it as the third line then it finds the form okay? Here is my code:
<?xml version="1.0" encoding="UTF-8"?>
<xfdf xmlns="http://ns.adobe.com/xfdf/" xml:space="preserve">
<f href="/ACORD/137nc.pdf"/>
<fields>
<field name="Form"
<field name="Date">
<value>05/23/2003</value>
</field>
<field name="Producer">
<Field name="FirstName">
<Value>Tracy Holzer</value>
</Field>
</Field>
</field>
</fields>
</xfdf>
However, now it does not read the fields correctly. The fields are: form.date, form.producer.firstname, etc...
