As a cheap and cheerful approach, if you really only need something that simple, you could just rig up a switch to deliver 5v to either the CTS or CD lines of the serial port and monitor the status using MSCOMM. On the same principle you can use the DTR (and RTS?) pins to set external states - although you may need a relay to manage AC power switching.
