I've cited it before and can't find it right now.
It was a position by Wolfowitz when he was NSS Advisor (I believe) in the Bush I administration. It was deemed too dangerous/controversial at the time and so was watered down to fit the times.

But when 911 happened, Wolfowitz was there with the old plan and it has all appearances of being carried out today.
