Try a Microsoft Windows driver. You may loose features of the present card but this is only a test at this point. Your card has some interesting features. If using a plain Windows driver does not cure the problem I would substitute an inexpensive video card that does not support dual output to see the effect.

Being an electronics engineer and having worked with computers since 1962, I can think of many possible problems. Do you have another computer you can use for testing?
