If you are interested in a EULA that nullifies your responsability (and almost makes the enduser have to pay you if it doesn't work). Look at several Microsoft EULAs. The EULAs for unsupported programs like regclean are a good start.

As far as the license agreement goes, I wonder if I could just put in a liability statement saying that "I am in no way responsible for any damages that may result in the use of this software." and make sure I receive an email from the customer verifying their agreement to the license. I don't know if this would fully protect me or not, but I would hate to get lawyers involved to draw up some contractual agreement. As you can tell, I'm cheap and I'm always looking for the easy way out :)
