Unless you need some feature that Citrix provides, there isn't any need to run it. Citrix runs on the top of Terminal Server and adds features - but Terminal Server has been greatly enhanced for things like printing, local drive mapping, etc.
I absolutely would not try and run the app over the VPN - you are describing a connection that could be as slow as one tenth of one megabyte. I would not even run an exe over a 10mb (10base) local network nowadays. If you have a 4-5mb exe, it could take several minutes for the application to even start. Even if each site had a T1 connection at 1.5mb, you are still only running at one tenth the speed of a 10mb network. Do yourself a favor, and don't try it.
The situation you describe is PERFECT for a terminal based solution.
