Mario, several of us have run into it. One client found it was hardware/driver related. On one machine they had a bad network card. On another they were using HP5 printer drivers. When they re-installed HP4 drivers (can be used by the HP5 with no problem) the ISO problems went away.
If you're on a network, look to see which workstation FIRST shows the problem after you've closed everyone out and re-indexed. That's probably where your problem is.
