I've seen this happens when the name of .PRG and the name of FUNCTION/PROCEDURE in it doesn match, For example,

= myfunc()
...
* myfunc.prg
FUNCTION myfunction
...

The myfunc.prg will be included in the project but you'll still get error "Unknown MYFUNC - Undefined".
