After installing MM .NET, when you click the New Project button you should see the following folders at the top of the Project Types pane (without manually adding them yourself):

MM .NET Visual C# Projects
MM .NET Visual Basic Projects

Are you using MM .NET 2003 with Visual Studio .NET 2003? It's specifically targeted for this version of VS .NET and version 1.1 of the .NET Framework!
