we have a similar problem
solution each item gets an uniq ID
it ist stored in a tabel with 7 fields
ID
ID of Question
value
info 1..
info 2..
when you want to ad a new item you just a a new ID and there is no need to redesingn the database
when you want a display just requery for the propper ID Numbers
we had a hard time when we started the redesingn thinking but now we never need to change a database structure if the project needs mor information
