Have you selected Customize... and editted the debug toolbar? It's not exactly intuitive, but you can drag a button off the toolbar to remove it, and drag/drop one from the customize dialog to add it to a toolbar.

>Maybe I'm crazy, but it seems to me there used to be a cancel button on the Debugger toolbar, where I now have a resume button. I'm certain that I've cancelled execution by clicking a button before, but now it seems to be gone, and I can't find anything under Options to bring it back.
