It's quite hard. DICOM files don't have a standard header - they contain a variable amount of text at the beginning of the file. Usually, the software working with them recognizes the file only by extension.
I had to manage them once, and the only way I found feasible was to convert them to another format (high resolution JPB - it was ok for screening).
