If you drop OLEContainer onto a form, then select your ActiveX in the listbox, the editbox below will tell you the filename of the DLL/OCX that contains the control.

You can also get to the information from the ObjectBrowser. Select the ActiveX under the Classes item and the bottom of the dialog will show Member of XXX where XXX is a clickable link, click that link and you'll see the OCX/DLL name.
