Eric,

In addition to what Steven told you, the icon file should have both 16x16 and 32x32 icons in it. If it doesn't the foxhead will show up in the Windows Explorer (and open file dialogs), rather than the application's. There's an article here in the knowledge base (Project Manager) that will tell you how to do create such a file.
