I had a similar problem only when I started using MS Word XP to crate HTML files to be the source for CHM File using HTML Help Workshop.

I noticed upon inspecting the generated HTML code and wasting a day or so that the 'imagedata src' line had a different graphic file than the 'src' line when I made them match, all graphics showed up again.

You can use this workaround if you still use MS Word for authoring HTML files.
