SQL Server is also using UDP 1434 for the SQL server discovery service. You might want to let this one through, as well.

Do you have by chance the Named Pipes network library enabled on the SQL server? This would indeed cause NETBIOS traffic. Rather then opening up NETBIOS I'd remove the network library in the SQL Server Network Utility.
