Why don't you live names as they are to see if it works?

It was easier in Windows98 to create it manually.... :)

The FAQ provides info how to do that both ways. There's reference to mskb #Q157172 How to Create Custom Forms in Windows NT 4.0 and Windows 2000 that explains how to create form manualy.
