I suspect that your icon file only has the 32x32 pixels size in it. It needs to include the 16x16 pixels size icon in it also. You will need an icon editor to do this. The basic steps are to open the icon in the editor. DO a copy on the 32x32 image, create a new image in the same ICO file but with a 16x16 size. Then paste the 32x32 sized image into the new image. Then save the ICO file. Practice this on a COPY of your original ICO file.
