This is kind of a late response. but I just stumbled across this thread and it seems like it has been not totally resolved. Have you looked at PDF Facory (www.pdffactory.com)? I recommend this to my clients and it doesn't involve any integration as it is set up as a printer option (much as Acrobat writer). Also, you can easily combine multiple page formats from multiple applications if you like. There is a freeware version, but I am sure they preferyou to buy (@$40-$50 per seat). My clients have nothing but good things to say about this product.
