Heading tags H1, H2, H3, etc needed for automatic contents. See mk:@MSITStore:C:\WINDOWS\Help\htmlhelp.chm::/cntHtag2.htm

Keyword links (KLinks) needed to include keywords from HTML files. See mk:@MSITStore:C:\WINDOWS\Help\htmlhelp.chm::/htklink2.htm

IMHO automatic contents and index is not very useful. You almost cannot control it.
/A new technology turns into completely outdated stuff before you have a time to read "Getting Started..." section.
/If there are some "system programmers" then others are unsystematic.
