You are correct in that each provider's visit is considered a separate incident, however, we do have several common links such as the patients current medication info, demographic information, home caregiver's, contacts etc. that have the possiblity of being modified during each visit.

An example would be previous Doctor orders modified for the patient based on the outcomes of each visit and these are across disciplines.

In most cases we do not have a problem, but there is always the exceptions that have made things difficult.
