I know this is a topic everyone has to deal with, even when you are working alone you must deal with changes and product specification.

I am curious to learn what other development shops are doing for project management:

-How do you keep tabs on a project between team members?
-How do you set specific goal posts and report on each accomplishment?
-How do you handle project changes?
-Are there any techniques you use?
-How do you receive bug reports and track their fixes?
-How about project management software?
-Any recommendations on software?

Our shop is suffering from growth. We have been spread thin too fast, and we need your assistance in offering what you do to maintain a sane shop. (I guess I would like to hear what does not work too, that may have some value as a way to recognize the problems).
