You are not able to test the icon for the EXE. :-)
It's not the same as the sample code.
But it is 16x16 and 32x32.

When I run the EXE, and press ALT+TAB, all forms have correct icons.
Except for forms not showing in the taskbar.
The icon for these forms is a Windows icon.
It looks like the "Printer test page icon" with a lot of dots to the left of the flag.
