under the "C:\Program Files\Mere Mortals .NET Framework 2003\Samples\MM Northwind Sample Application_Visual Basic" the only forms available are:

CustomerOrderForm
EmployeeForm
MainForm
UserLoginForm

I think that i'm missing some forms even the code when you into it references forms that are not available like the form you mentioned above. If you could forward a copy of an updated sample file with all the forms in it i would be most thankful.
