I would do some spot check verifications with the credit card center and so on. The best bet is that you'll end up with a charge back extremely fast on that one. So, you might want to counter that one before it hits back. From all you mentioned, this looks extremely suspicious. We've seen that a few times here and we simply close the door immediately at various levels.
