I suspect that you still have a reference somewhere to the form that prevent that form from releasing. Are you passing the reference to the form around? If yes, make sure that they NULLify the reference when they are done with it. If not, there may be some code in the QueryUnload event (NODEFAULT?) that prevent the form from releasing.
