I've seen this when a user closes by the "X" rather than the Quit menu option. Closing through the window executes the ON SHUTDOWN which in my case does not traverse the open tables doing a TableRevert on them. I haven't had time to fully explore this, but it's definately at the top of the list of items to check when I do. I use stored procedures too although none could possibly be running at the time of ON SHUTDOWN execution. Let me know if you do experiment with this and what you come up with.
