I have used omnipage ocr (and most of its competitors by the way).
Clearly it is the winner in terms of accuracy and perfs.

If you look for effective OCR, you should be aware however that this technology (OCR)n though very demanding, still offers poor results for handwritten stuff. Even in 2004.

If you wish to convert clearly defined "form-based fields" into proper input, then you should possibly look into dedicated "form management software".

I have not tested these tools however.
