It was something really simple, and included some features specifically for dentists. Really not worthwhile to send screenshots (I would have to get it to work first <g>, since I have been doing some changes which I shouldn't have.)
I would basically put a grid on the top half of a form, for selecting records, and fields for editing the current record on the bottom half.
Fields for the current day are shown; a date selector allows the user to change to another date. Or perhaps allow a range of dates, too.
Really, nothing fancy, as you can see.
