Sorry, I probably confusing you with someone else's response on this site - I think someone else said they did something like this.

I also found the following document when I went to download the trial copy of 6.

It explains the various products: "Adobe Designer 6" is now the replacement for Adobe Form Designer 5.0 and also the suggested update from Adobe Output Designer 5, which seems to be the name of a JetForm product that they have now shelved....if I read this correctly - Albert

We submit the forms to the carrier and to the board and once approved use them. When we purchase the forms from ACORD they are already approved.
