Custom pages in XP are now setup in the Printer and Faxes applet. You just go to Start/Settings/Printers and Faxes. Then go to File / Server Properties. In the first page you will find a check Create New Form, select it and define the size of your new custom form. Give it a name, then click on Save Form.
