Here are things that could be prompted for (optionally)

1) Web Forms or Winforms
2) C# or VB.NET (or other language)
3) Back-end datasource (VFP, Oracle, SQL Server, etc.)
4) Edit box for code sample (or at least a label encouraging people to provide a code sample, if applicable)
5) Operating system (XP, 2000, 98, etc.)
6) Full .NET error message (if applicable)
7) A screen shot? (someone could attach a JPG or BMP…sometimes just ‘seeing’ what the person is trying to do can be helpful...I realize that's a significant change, but figured I'd mention it)

I realize that some of these are represented as sections - though a single post might contain enough detail that it could possibly touch on several sections. Like an ADO.NET question in C# that relates to a back-end issue, etc.
