Javier he visto este tipo de comportamiento antes y generalmente se debe a problemas con el driver. Generalmente el problema es con el ancho del papel asi que lo que generalmente hago es poner el ancho un poco más pequeño y problema resuelto pero como dices que el ancho que definiste es solo 4.5" no creo que haya problemas con eso. Intentaste cambiando el driver de la impresora?
