Under WIndows 2000/XP custom paper size is handled differently. See mskb #Q304639 HOWTO: Find PaperSize for Custom Print Sizes Under Windows NT and Windows 2000 and mskb #Q157172 How to Create Custom Forms in Windows NT 4.0 and Windows 2000 for details.
You can also do that programmatically, using my FAQ's Enumerating printer forms under Windows NT/2K/XP FAQ #22445 and How to add/delete custom printer forms under Windows NT/2K/XP FAQ #21202.
