If your talking about not allowing 1 user to have more than 1 instance open on their machines then I would suggest a check on running the program which looks for an open window of the same name as your application.

If your talking about only allowing 1 instance throughout an organisation then I would suggest the application locks a record in a separate table and the application checks to see if it can lock this record on the way in.
