I saw a senior on-site (in Fallujha) military man discussing the "mop up" operations there. He said, quoting roughly 'There are still some who want to fight to the death and we aim to accommodate them'.
Nice sound-bite, but it may also provide some idea of the "orders" that were operating at the time.
