Alright, I solved #2.

Turns out Adobe Acrobat Standard does not support editing FDF forms. I upgraded to Acrobat Professional and I can now go to Tools->Advanced Editing->Forms->Show Forms Toolbar. On this toolbar I can click on the text field tool button and all my fields display.
