With the help of Dimitry, the cause of my problem was found to be that I had outlook "Mail Servers" configured to "Internet Mail Only" instead of "Corporate or Workgroup." As soon as I switched to the other mode, it worked perfectly.

It appears that "Internet Mail Only" mode breaks Extended MAPI.
