Algo que podrías verificar es si el índice por el campo código tiene un filtro, ya que Rushmore ignora los índices con filtro, además asegurate que el indice sea realmente sólo por código, y no por UPPER(Código) o alguna otra función, ya que de ser así deberías applicar la misma función al SQL
