In addition to what Sergey and Garrett have said, you might want to take a look at the paper http://www.ita-software.com/papers/Borup_WindowsInstaller.pdf. In particular, the section on Upgrades, Updates, and Patches (which starts on page 11) will help you understand what's going on when you deploy an update to an application using a Windows Installer-based product like InstallShield.
