No comments on the combo issue, you seem to be doing the right thing. It's this:

I wouldn't ever let the doctors type the disease names by themselves. There's the WHO official table of diseases, I think revision 10 is active et the moment, and my logic was always to have them pick from it into their private tables (and then pick from there), and it's the disease codes which actually get into tables.

Yes, there's always a memo with the disease name copied and they can edit it as they choose, but I think having an official codes is essential, because you know how doctors love statistics :). And so does the health insurance (or social security or whatever be their local name).
