It's a Red White and Blue "Air Bill" form that looks (looked?) just like the multi-copy forms (with the perfed edges) that FEDEX provides (provided?) it's pick-up clients. Has a file that contains bill to-ship to info. It's just a GUI - I wrote for a friend (for fun) - but he went out of business.

My suspicion is that FEDEX problably has an on-line form - the only advantage of this one is that it keeps a database of ship-to accounts and numbers.
