A comforter is somewhere between a quilt and a bedspread. In my experience, a bedspread is meant to be a decorative covering only and is supposed to be entirely removed before you go to sleep. A comforter is decorative, but is typically quilted and adds a layer of warmth.

I've had a duvet in a hotel or two. The big issue for me is that I like something to be tucked in at the foot of the bed.

As I said <g>, I like something tucked in at the bottom, but if the bed is made up properly in the first place (good hospital corners), the sheet and blanket stay tucked, so making up the bed in the morning is just a matter of pulling the covers up neatly.
