Most common of all IMO is that the person who did the interview, and has the authority to say yes or no, has their regular job to do and is caught up in some crisis there. You have to keep in mind that most interviewers, beyond the HR screening stage, fit it in while doing their main job. Guess which comes first? Obviously the hiring task will be prioritized when it's a priority position, but not all openings are for priority positions.

You're missing the point. It's not about interviewed person expectations. They promise to give their decision next day and didn't keep the promise.
