I remember seeing a 60 Minutes (or one similar) segment where a person investigated the details of their hospitalization bill (U.S.).
It was fan-fold paper and was at least 25 pages of single-line items.
The one I remember... One item was repeated many many times and was $5.00 for each occurrence. It had some fancy name that I don't remember. But it was something akin to "mucous removal device". Turned out that any time a nurse wiped the patient's nose/mouth with a kleenex a charge was added to the bill.
