You asked for an example, I provided one, and now you seek to negate it by altering it to remove its point. ;-)

I'm not altering it arbitrarily - I just think the example was contrary to what usually happens.

When I visit a doctor for the first time, I fill out a form and it is entered by a single clerk. Or I sit beside a clerk and he/she asks me questions and then types the answers. Sorry, I just didn't see anything 'real-world' in the example.
