It sounds familiar :-)

These are my thoughts:
- are you using any third party tools or frameworks (Mere Mortals, StrataFrame, ...) and didn't include them in your setup project?
- are you accessing some (system) protected file / folder in your startup program that the client doesn't have access rights?
- are you creating a hardcoded file (eg: c:\temp\testfile.txt) in a folder / drive that doesn't exist or the client has no access rights?
- ...
